Our Commitment

As home care professionals, we
understand the issues involved in the care of a patient in their home. We respect the need for privacy and the need for open, accurate communication concerning their care. To meet these needs, our commitment is to:
• Develop a care plan based on the individual needs of the patient and the orders prescribed by the physician.
• Coordinate all ancillary services not directly provided by our reach.
• Maintain a written multi-discipline communication log, in the home, documenting the patient's progress
towards meeting their care plan goals.
• Provide on site and telecommunication supervision of all clinical staff to ensure that the goals of the care plan are met and that the patient is satisfied with the services provided.

OUR THERAPIST evaluate each patient's physical impairments and functional limitations and establish a plan of care in cohesion with the patient's physician in order to acheive optimal rehab outcomes.
OUR THERAPIST can offer:
Physical therapy service, assist in restoring and maintaining maximum musculoskeletal functions. From hip fractue to spains, to patients with unsteady gait, our therapist are specialized in helping the patient restore thier confidence intheir ability to ambulate themselves throughout their living environment.
